Student Registration

Northern Lehigh School District

When and Where:

Registration Hours are Monday through Friday 8-11am or 1-3pm, at the District Office.

The following items MUST be presented at the time of registration:

• Two forms of CURRENT proof of residency (one from each list):
  1. Agreement of Sale, Deed, Occupancy Permit OR Rental Lease Agreement
       AND
  2. Moving Permit, Current Utility Bill, Current Tax Bill/Receipt, Current Bank Statement/Pay Stub, Vehicle Registration Card, Health Insurance Card, or Letter from Employer
• Original Birth Certificate
• Immunization (shot) record
• Transfer card from current school district
• IEP (if applicable)
• Custody Documents (if applicable)

Also, The following forms are required:

• Registration Form Packet
• Health Information and Immunization Records
• Transportation Form
• Student Withdrawal Form for Withdrawals ONLY
